Families and Communities Support Model

Develop a place-based system of hubs, personal casework, integrated pathways and outreach support providing a single co-ordinated experience of access to financial empowerment, housing support and pathways to safe and affordable homes, food sharing, health, employability and family support. This system change aims to ensure people receive the right support at the right time, and seeks to reduce health inequalities and the risk of poverty and homelessness, while preventing avoidable harms such as poor mental wellbeing, domestic abuse and substance use.
